To enroll in one of Puget Sound Health Partners plan you have four options:

1. Enroll Online through Medicare.gov 
2. Call 1-866-789-7747 and one of our Representative can assist in enrolling you over the phone
3. Request our Free enrollment kit and mail back the enrollment form 
4. Print out and mail the online enrollment form
