Medication Therapy Management Program
PSHP's Medication Therapy Management Program (MTMP) identifies beneficiaries who are at risk for adverse medical or drug events. The MTMP triggers actionable interventions to physicians to review therapy, and encourages compliance with appropriate dosing, length of therapy and national, evidence-based clinical guidelines.
Beneficiaries who meet the following criteria are automatically enrolled in the MTMP:
- Have three or more targeted chronic diseases:
- Alzheimer's
- Asthma
- Behavioral health
- Diabetes
- Cardiovascular disease
- Heart failure
- Osteoporosis
- Are taking at least eight covered Part D drugs; and
- Are likely to incur annual costs for covered Part D drugs in excess of $3,000 for 2010
Beneficiaries participating in this program benefit from receiving the following education regarding common health-related issues:
- A quarterly newsletter
- One-on-one personalized telephonic counseling with a pharmacist
