PSHP has established several programs to help you through challenges with your health. Our programs are centered around you, helping you maintain your good health and stay on track with your health care needs. We accomplish this by working in a close partnership with your Primary Care Provider.
Care Management
Care Management is a comprehensive method of client assessment which is designed to identify client vulnerability, needs, and goals. The goal is to provide client advocacy, a system for coordinating client services, a systematic approach to evaluation and most importantly, to address your needs foremost.
You and your loved ones may benefit from care coordination if you suffer from an illness that requires daily services or care. If your health problems require coordination of multiple services PSHP will help to ensure that you receive appropriate care in the appropriate setting.
Adult Population Health
Health Risk Assessment Survey: The Health Risk Assessment Survey is a tool that asks questions about your medical history, lifestyle, medications and more. PSHP shares the completed survey with your Primary Care Provider to help ensure you are receiving care based on your needs. In addition, PSHP relies on your survey responses to to continually improve our programs and services for you.
If you have not completed a Health Risk Assessment Survey in the last 12 months please click here to print out and mail in your completed survey. The results are confidential and will only be shared with you personal physician. Link to online HRA
Quality Initiatives
Survey Sources: PSHP collects standardized information on clinical and service performance measures to assess how we are doing in areas important to our Member's care. PSHP uses three standard national survey processes; Healthcare Effectiveness Data and Information Set (HEDIS), Consumer Assessment Health Plan Services (CAHPS) and Health Outcomes Survey (HOS) to determine how we are doing, how we compare to our others and to guide us in continually improving performance outcomes. PSHP will share information with you on important survey results through our website and other communication venues.
Member Resources & Patient Safety
There is a wealth of information and resources publically available on a wide variety of health topics. Some of this information is reliable and some is questionable. When it comes to your health and well-being your Physician's Office should always be consulted.
PSHP has established a link to connect you with current and reliable Patient and Consumer information. The resources here are intended to inform you as well as guide your communications with your Physician and Health Care Team.
